Additional information for attendees being wholly or partially funded

Information required for purchase of airline tickets and reimbursement of expenses:

• Full name as it appears on your identification (passport or driver's license)
• Social Security Number
• Complete mailing address
• Work and home telephone and fax numbers
• Airport name you are departing from and returning to
• Tickets will be purchased for arrival Sunday, Feb. 27 and departure Wednesday, March 2 after 8:30 p.m. or Thursday, March 3. Cost will be the main consideration in purchasing airline tickets.
• If an airline ticket is being purchased for you, you may wish to include seat preferences and all mileage plan numbers

Reimbursement of expenses

Lodging: only room costs and applicable taxes will be reimbursed.
